A toolkit for knowledge translation and exchange (KTE) planning developed by the Fraser Health Authority in British Columbia, Canada. The toolkit was developed with the objective of enabling the end-user to be able to “acquire, assess, adapt and apply evidence.” It is anticipated that end-users of the Toolkit will include clinicians, decision-makers, evaluation and research teams, and anyone interested in the transfer and exchange of knowledge. The toolkit is divided into four sections: The Basics: Useful Tools for KTE; KT Casebooks: numerous electronic links to KTE resources.
